Job Description

In Fall 2017, The Clearity Foundation launched a collaborative Ovarian Cancer Pilot (OCP) project to revolutionize the ovarian cancer journey for patients and families. Central to the project is a suite of human, electronic, on-line and organizational resources including: delivery of telehealth psychosocial support by licensed Oncology Social Workers; creation of a Content Library containing trusted, curated and accessible information; design of Distribution Channels with health systems, advocacy organizations and direct marketing for patient recruitment and specialty referral; enhancement of Clinical Trial Matching; and Evaluation to assess process objectives and patient outcome measures.

As part of this project, TCF is recruiting an experienced Lead Oncology Social Worker to join our Team and deliver psychosocial support through a new phone-based program. The position involves a minimum of 30 hours per week with range up to full time and is located at the offices of the Clearity Foundation in San Diego, California.

Qualifications

Licensed Clinical Social Worker credential required in the State of California or license within 3 months of date of hire. Minimum of 3 years as Clinical Social Work experience operating in an oncology setting within a hospital, ambulatory clinic or social service organization. Certification or certification eligible as Oncology Social Worker (OSW-C) by the Board of Oncology Social Work (BOSW). Membership in the Association of Oncology Social Work (AOSW).

Scope of Responsibilities

• Clinical Services – Provide initial and ongoing OSW assessment with related clinical psychosocial OSW support services to women facing ovarian cancer and their caregivers involving various telephone call types: Appointment-based; Open Chat Line; Message Return; Family Conference; and Q & A Phone Groups.
• Program Development ​– Provide feedback and input into ongoing development of new program model of patient support. Demonstrate commitment to and passion for the Clearity Foundation and the Ovarian Cancer Project’s mission and goals.
• Collaboration – Serve as a member of patient care team and participate in regular meetings to ensure best patient outcomes, including: Team Case Review Tele/Video Conferences.
• Documentation – Complete ongoing documentation of daily interactions including assessments, treatment plans and progress notes, while maintaining confidentiality and privacy mandates.

Job Description

The Patient Services Associate will assist patients and physicians who participate in The Clearity Foundation* Tumor Profiling Assistance Program by preparing test requisitions and coordinating with the testing laboratories. Additional administrative tasks include medical record requests, receipt and electronic storage, as well as database entry. Individual will be an excellent and compassionate communicator, who is motivated, detail- and task-oriented, and proficient with computer programs (Microsoft Word and Microsoft Excel). Familiarity with medical records and experience with database environments is strongly desirable.

• Associates Degree or greater
• Excellent and compassionate communicator
• Motivated, detailed, and task oriented
• Proficient in Microsoft Word and Microsoft Excel
• Familiarity with medical records and knowledge of cancer biology are strongly desirable.
